What if UPSI is shared with many people at the same time? What should I record in the Structured Digital Database – one consolidated entry or each separately? 

The regulations are not clear on this. Regardless, Affinis(SDD) allows you to simply create a single entry which will be split into multiple entries with all the appropriate details.
